In order to be able to assess the success of a program, evaluation methods should be included in the proposal. The following are examples of program evaluation tools that can be helpful in evaluating your program:
Nutrition education evaluation resource:

Outcome evaluation resources:
A survey model and questions on eating and physical activity patterns for teens can be obtained from the Youth Risk Behavior Survey that provides national comparisons and previously-tested questions: (CDC Full Report - PDF)
Please note that since the Youth Risk Behavior Survey was written for teens, it should be adapted for use with younger children. For grades 1-5, questions might need to be read to the children. When assessing kindergarten and preschool children, the questions should be answered by caregivers or parents.

Note: In addition, to the program evaluation methods submitted in your grant application, all funded programs will receive a specific evaluation form to complete as part of the grant evaluation process.
